During participation in Hastings Catholic School (HCS) activities and events, students and student products are often photographed, videotaped, and/or audiotaped. They are sometimes published in educational, instructional, and informational materials for HCS. The purpose of these materials is to showcase student work and participation in publications. Publications might include: St. Cecilia Middle and High School (STC) Yearbook, STC’s news (Cecil’s Chatter), STC Live, STC Online, HCS brochures and literature, HCS Links, Hastings Catholic Schools Alumni & Development Office literature and website. The goal is to positively recognize students and share the HCS experience.

I hereby give my consent for my student and his or her work to be photographed, videotaped, or audiotaped. I also waive any right I may have to inspect and/or approve the material, and release HCS from any and all liability which could result from its use. I further authorize HCS to use the above mentioned material in whole or in part, in any manner that they deem appropriate, including any reproductions thereof and to identify students appearing in reproduced material by full name in hard copy publications and by first name and last initial only in website publications.

This form will remain on file and in effect for the duration of participation at HCS. Date: __________________
